Investing in digital health is one of the most impactful ways to advance healthcare access. Digital health technologies break down traditional barriers to care, particularly for underserved communities.

Our Challenge

Parkland Health primarily serves people facing significant economic, social and health-related barriers. Patients report lack of transportation, inability to take time off work, lack of childcare and limited mobility as the challenges preventing them from accessing critical healthcare services. 

Our Solution

Digital healthcare technologies such as telehealth, remote patient monitoring and AI-powered solutions help maximize caregivers’ time management and ensure timely care for patients. 

Parkland Connect

Parkland Connect is a telehealth clinic through which patients can connect with a physician for a video or phone visit, allowing them to get care anywhere, at a time convenient for them. For many in our community, Parkland Connect means the difference between delaying care and getting timely treatment. We seek to expand the team and add video health pods located at our trusted community-based partner organizations. 

Hospital at Home

This patient-centered healthcare model blends high-tech and high-touch services to provide hospital-level care to eligible patients in their homes instead of a traditional hospital setting. Care includes physician and nurse visits, labs, IV medications and 24/7 nurse monitoring. The Hospital at Home program reduces healthcare costs, frees up inpatient beds for more acute patients and helps decrease hospital readmissions. Parkland plans to expand this service.

Remote Patient Monitoring

Remote patient monitoring (RPM) enables continuous, real-time oversight of patients outside traditional clinical settings. It plays a critical role in managing chronic conditions, reducing hospital readmissions and improving early intervention for at-risk populations. Parkland is currently utilizing remote patient monitoring for patients with hypertension, heart failure and issues requiring pulmonary rehabilitation. We intend to expand the service for hypertension and add service for our diabetic population of patients. 

Delayed Care Surveillance

Delayed care surveillance enables Parkland staff to detect barriers to care and respond with targeted strategies to ensure timely, equitable access. AI is used to identify patients who have been referred for a follow up appointment but have not scheduled and/or shown up for their appointment. Rather than leaving this patient untreated, which could result in further decline of their health or a costly visit to the emergency room, they are personally contacted to determine and provide a solution for the cause of their delay in care. 

Ambient Listening

This project uses AI-enabled microphones and software to capture and transcribe clinical conversations between providers and patients in real time. The goal is to reduce documentation burden and stress, improve accuracy and enhance the patient experience. Ambient Listening will be expanded to all providers at Parkland as funding becomes available.
